How to approach it

Choose a real mistake with real consequences -- not something trivial and not something that was clearly someone else's fault. The most compelling answers involve a mistake that taught you something you still carry with you.

Be honest about what went wrong and why. Take clear ownership rather than hedging or distributing blame. Describe what happened when the mistake was discovered and the immediate steps you took to address it.
- Hint 3
Focus heavily on the aftermath: what did you learn, how did it change your approach, and what systems or habits did you put in place to prevent similar mistakes? The learning and growth should be the longest part of your answer.
